California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care plans with healthcare teams, and ensure access to necessary services while working remotely.

Key responsibilities:

• Evaluate patient needs and develop client-focused case management plans in collaboration with healthcare teams
• Coordinate resources and services to ensure continuity and quality of care, including monitoring patient progress toward treatment goals
• Assist in identifying barriers to care and facilitate referrals to appropriate services and resources

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and working with healthcare teams
• Knowledge of regulatory and accreditation guidelines relevant to case management
• Ability to document and maintain accurate patient records
• Familiarity with community resources and services available to patients and families
