California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate needs, coordinate care plans, and ensure continuity of services while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop client-focused case management plans
• Monitor patient progress towards treatment goals and assist in identifying barriers to care
• Document patient cases and ensure adherence to regulatory guidelines and standards

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and making referrals for patients
• Knowledge of regulatory, accreditation, and regional guidelines
• Ability to collaborate effectively with healthcare teams and patients
• Experience in documenting patient cases and managing authorization data
