California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Assess patient needs and eligibility for services, ensuring compliance with regulatory guidelines
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams and caregivers
• Monitor patient progress towards treatment goals and coordinate resources to ensure continuity of care

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and developing case management plans
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to work collaboratively with healthcare teams and communicate effectively with patients and caregivers
• Familiarity with patient assessment and monitoring processes
