California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities

• Evaluate patient needs and coordinate resources to ensure continuity and quality of care
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams and caregivers
• Monitor patient progress toward treatment milestones and assist in identifying barriers to achieving goals

Required qualifications

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in collaborating with healthcare teams and coordinating patient care
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to assist in the development of client-focused case management plans
• Familiarity with authorization processes and documentation standards in healthcare
