Job Summary
Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.
Key responsibilities:
- Coordinate with patients and caregivers to assess needs and determine eligibility for services
- Develop and monitor comprehensive case management plans in collaboration with healthcare teams
