California Licensed Case Management Coordinator

Job Summary

Providing case management services for a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop case management plans
• Monitor patient progress toward treatment goals and identify barriers to care
• Document patient cases and ensure compliance with regulatory guidelines and standards

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in evaluating patient needs and coordinating care services
• Knowledge of healthcare regulations and accreditation standards
• Ability to develop and communicate comprehensive care plans
• Experience in documenting and managing patient cases effectively
