California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluates patient needs and coordinates resources to ensure continuity and quality of care
• Develops and communicates comprehensive disposition plans in collaboration with healthcare teams and patients
• Documents patient cases and monitors progress toward treatment milestones and care plan goals

Required qualifications:

• Bachelor's degree in Nursing or a related field OR a minimum of three years of experience in case management or a directly related field
• Experience in developing patient care plans in collaboration with healthcare teams
• Knowledge of regulatory, accreditation, and regional guidelines for case management
• Ability to assess medical necessity for hospital admissions and required levels of care
• Familiarity with community resources and services available to patients and caregivers
