California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, develop care plans, and coordinate resources while working remotely.

Key responsibilities:

• Evaluate patient needs and goals to determine eligibility and develop client-focused case management plans
• Coordinate resources and services to ensure continuity and quality of care, attending case management rounds as needed
• Document patient cases and assist in disposition planning activities in collaboration with healthcare teams

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing and implementing care plans for patients
• Knowledge of healthcare regulations and standards related to case management
• Ability to coordinate with multidisciplinary teams and external resources
• Familiarity with patient assessment and progress evaluation processes
