California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Coordinate resources and services to ensure continuity and quality of care for patients
• Assist in developing and communicating comprehensive disposition plans in collaboration with healthcare teams
• Document patient cases and monitor progress toward treatment milestones and care plan goals

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience with case management processes and regulatory guidelines
• Ability to collaborate effectively with healthcare teams and communicate patient needs
• Familiarity with patient eligibility assessments and authorization processes
• Commitment to ongoing professional development and adherence to healthcare policies
