California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities

• Evaluate patient needs and coordinate resources to ensure continuity and quality of care
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams, patients, and caregivers
• Monitor patient progress toward treatment goals and assist in identifying barriers to achieving these goals

Required qualifications

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of medical necessity assessments and authorization processes
• Experience in developing and implementing patient care plans
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with regulatory and accreditation guidelines relevant to case management
