California Licensed Case Management Coordinator

Job Summary

To support a growing healthcare team, the full-time California Licensed Case Management Coordinator will provide case management services to a limited caseload of low-risk patients remotely, coordinating care and developing client-focused management plans in collaboration with healthcare professionals and caregivers.

Key responsibilities:

• Evaluate patient needs and goals, determining eligibility and coordinating resources for continuity of care
• Develop and communicate comprehensive case management plans in collaboration with patients, caregivers, and healthcare teams
• Monitor patient progress towards treatment milestones and assist in identifying and addressing barriers to care

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing and implementing case management plans
• Knowledge of healthcare regulations and accreditation standards
• Ability to collaborate effectively with multidisciplinary teams
• Proficiency in documenting patient cases and managing authorizations
