California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care plans, and ensure continuity of services while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate with caregivers to develop comprehensive case management plans
• Monitor patient progress towards treatment milestones and assist in identifying barriers to achieving goals
• Document patient cases and ensure adherence to regulatory and accreditation guidelines

Required qualifications:

• Bachelor's degree in Nursing or related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and assessing patient eligibility and benefits
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with documenting patient cases in medical files
