California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Coordinate resources and services to ensure continuity and quality of care for patients
• Assist in the development and monitoring of individualized case management plans in collaboration with healthcare teams
• Document patient cases accurately and maintain compliance with regulatory guidelines

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in assessing patient needs and developing care plans
• Familiarity with healthcare regulations and accreditation standards
• Ability to work collaboratively with healthcare teams and external resources
• Proficient in documenting patient information and managing case files
