California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will coordinate patient needs, evaluate eligibility and benefits, and develop client-focused care plans in a remote setting.

Key responsibilities:

• Evaluate patient needs and goals, determining eligibility and benefits for admissions
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams and caregivers
• Monitor patient progress towards treatment goals and coordinate resources for continuity of care

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of healthcare regulations and standards
• Experience in developing and implementing care plans
• Ability to coordinate with multidisciplinary teams and external resources
• Familiarity with patient eligibility and authorization processes
