California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop comprehensive case management plans
• Monitor patient progress towards treatment goals and ensure continuity of care by coordinating resources and services
• Document patient cases accurately and assist in verifying that services align with established guidelines and standards

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in evaluating patient needs and coordinating care services
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to work collaboratively with healthcare teams and communicate effectively with patients and caregivers
• Experience in documenting patient cases and managing authorization data
