California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate with caregivers to develop comprehensive case management plans
• Monitor patient progress toward treatment goals and ensure continuity and quality of care
• Assist in identifying barriers to care and facilitate access to necessary services and resources

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and managing patient cases effectively
• Knowledge of healthcare regulations, accreditation standards, and community resources
• Ability to collaborate with healthcare teams and communicate effectively with patients and families
• Proficiency in documenting patient cases and managing authorization data
