California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care plans, and ensure access to services while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop client-focused case management plans
• Monitor and evaluate patient progress towards treatment goals and ensure continuity of care
• Assist in obtaining authorizations for services and support patients in accessing appropriate resources

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of healthcare regulations and accreditation standards
• Experience in developing and implementing care plans
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with patient assessment and eligibility determination processes
