California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate with caregivers to develop comprehensive case management plans
• Monitor patient progress towards treatment goals and ensure continuity and quality of care
• Assist in connecting patients with appropriate resources and services, making referrals as necessary

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing and implementing case management plans
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to collaborate effectively with healthcare teams and external resources
• Experience in monitoring and evaluating patient care plans and outcomes
