California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and develop comprehensive case management plans in collaboration with healthcare teams and caregivers
• Coordinate resources and services to ensure continuity and quality of care while monitoring patient progress toward treatment goals
• Assist in obtaining necessary authorizations and facilitate patient access to care and community resources

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in evaluating patient needs and developing care plans
• Knowledge of healthcare regulations and standards related to case management
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with authorization processes and patient disposition planning
