California Licensed Case Management Coordinator

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Management Coordinator will evaluate patient needs and goals, coordinate care, and develop client-focused case management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and determine eligibility for services
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams
• Monitor patient progress towards treatment milestones and address barriers to achieving care goals

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Experience in coordinating resources and services to ensure continuity and quality of care
• Ability to document patient cases accurately in medical files
• Familiarity with community resources and patient support services
