California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused case management plans while working remotely.

Key responsibilities:

• Coordinate resources and services to ensure continuity and quality of care for patients
• Assist in developing and monitoring patient care plans in collaboration with healthcare teams
• Document patient cases and ensure adherence to regulatory guidelines and standards

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in assessing medical necessity and determining levels of care
• Knowledge of regulatory, accreditation, and regional guidelines
• Ability to collaborate effectively with healthcare teams and patients
• Familiarity with community resources and referral processes
