California Licensed Case Manager

Job Summary

Providing case management services to low-risk patients, the full-time California Licensed Case Manager will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate care to develop comprehensive case management plans in collaboration with healthcare teams
• Assess patient progress toward treatment goals and address barriers to ensure continuity and quality of care
• Document patient cases and maintain compliance with regulatory and accreditation guidelines

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of medical necessity assessments and authorization processes
• Experience collaborating with healthcare professionals and community resources
• Familiarity with regulatory and accreditation standards in case management
• Ability to work independently in a remote environment while managing a limited caseload
