California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will evaluate patient needs, develop care plans, and coordinate resources remotely while ensuring adherence to regulatory guidelines.

Key responsibilities:

• Evaluate patient needs and eligibility for services, coordinating with caregivers and healthcare teams
• Develop and communicate comprehensive case management plans in collaboration with patients and healthcare providers
• Monitor patient progress towards treatment goals and address barriers to care as needed

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and resources for patients
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to document and maintain accurate patient records
• Familiarity with community resources and services available to patients and caregivers
