California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate care, evaluate needs, and develop client-focused management plans in a remote work environment.

Key responsibilities

• Coordinate with patients and caregivers to assess needs, eligibility, and services while documenting case information accurately
• Develop and communicate comprehensive disposition plans in collaboration with healthcare teams and monitor patient progress toward treatment goals
• Assist in connecting patients to appropriate resources and services, ensuring continuity and quality of care throughout the process

Required qualifications

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• California Licensed Case Manager certification
• Experience in developing case management plans and coordinating care
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to work collaboratively with healthcare teams and communicate effectively with patients and caregivers
