California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused case management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop case management plans
• Monitor patient progress towards treatment milestones and ensure continuity of care
• Assist in identifying barriers to achieving care goals and facilitate access to necessary services

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and services for patients
• Knowledge of regulatory and accreditation guidelines related to case management
• Ability to collaborate effectively with healthcare teams and external resources
• Proficiency in documenting patient cases and managing authorization data
