California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and determine eligibility for services
• Develop and implement comprehensive case management plans in collaboration with healthcare teams
• Monitor patient progress and ensure continuity of care by addressing barriers and documenting cases accurately

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Experience in coordinating resources and services for patient care
• Ability to assess medical necessity for hospital admissions and required levels of care
• Familiarity with documentation practices in medical files
