California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate resources to ensure continuity and quality of care
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams and patients
• Monitor patient progress toward treatment goals and assist in identifying barriers to achieving these goals

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing patient-focused care plans
• Knowledge of regulatory, accreditation, and regional guidelines
• Ability to collaborate effectively with healthcare teams and patients
• Proficiency in documenting patient cases and managing authorization data
