California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Evaluate patient needs and coordinate resources to ensure continuity and quality of care
• Develop and communicate comprehensive disposition plans in collaboration with healthcare teams
• Document patient cases and monitor progress towards treatment milestones and care plan goals

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in developing and implementing case management plans
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with patient assessment and eligibility determination processes
