California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to assess needs, develop care plans, and ensure access to necessary services in a remote setting.

Key responsibilities:

• Evaluate patient needs and develop client-focused case management plans in collaboration with healthcare teams
• Monitor patient progress toward treatment goals and coordinate resources to ensure continuity of care
• Assist in identifying patients ready for disposition planning and communicate comprehensive plans with stakeholders

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating with healthcare teams and managing patient care plans
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to document and maintain accurate patient records in compliance with established standards
• Familiarity with community resources and services available to patients and caregivers
