California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate evaluations, develop client-focused care plans, and ensure access to necessary resources while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs, goals, and current services, determining eligibility and benefits for admissions
• Develop and communicate comprehensive case management plans in collaboration with healthcare teams, patients, and caregivers
• Monitor patient progress towards treatment goals, identifying barriers and ensuring adherence to regulatory guidelines

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating resources and services to ensure continuity and quality of care
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to document patient cases accurately in medical files
• Understanding of the healthcare industry and patient disposition planning processes
