California Licensed Case Manager

Job Summary

Providing case management services to a limited caseload of low-risk patients, the full-time California Licensed Case Manager will evaluate patient needs, coordinate care, and develop client-focused management plans while working remotely.

Key responsibilities

• Evaluate patient needs and determine eligibility and benefits for admissions, documenting all relevant data
• Coordinate resources and services to ensure continuity and quality of care, attending case management rounds and monitoring patient progress
• Develop and communicate comprehensive disposition plans in collaboration with healthcare teams, patients, and caregivers

Required qualifications

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating care and managing patient caseloads
• Knowledge of regulatory, accreditation, and regional guidelines related to case management
• Ability to document patient cases accurately and maintain medical records
• Familiarity with community resources and referral processes for patients
