California Licensed Case Manager

Job Summary

Providing case management services for a limited caseload of low-risk patients, the full-time California Licensed Case Manager will coordinate with patients and caregivers to evaluate needs, determine eligibility, and develop client-focused care plans while working remotely.

Key responsibilities:

• Coordinate with patients and caregivers to assess needs and develop comprehensive case management plans
• Monitor patient progress toward treatment goals and ensure continuity of care by collaborating with healthcare teams
• Document patient cases and assist in identifying barriers to care, ensuring adherence to regulatory guidelines

Required qualifications:

• Bachelor's degree in Nursing or a related field, or a minimum of three years of experience in case management or a directly related field
• Experience in coordinating patient care and managing case documentation
• Knowledge of healthcare regulations and accreditation standards
• Ability to collaborate effectively with healthcare teams and external resources
• Familiarity with developing and implementing care plans based on patient needs
