California Licensed Case Manager Nurse

Job Summary

To support patient care in the California market, the remote California Licensed Case Manager Nurse will complete telephonic nursing assessments, create care plans, and coordinate care while working full-time hours in accordance with PST business hours.

Key responsibilities:

• Conduct telephonic meetings with patients and families to discuss care and treatment plans
• Coordinate post-discharge care, ensuring follow-up needs are met for patients
• Document patient information and care management plans in the Electronic Health Record system

Required qualifications:

• Graduation from an accredited school of nursing
• Active, unrestricted Registered Nurse license through the State of California
• 1+ years of experience in a clinical setting or care management
• Intermediate level experience in Microsoft products including Word, Excel, and Outlook
• Ability to work PST business hours
