Job Summary
To support patients navigating complex diagnoses, the full-time California Licensed Case Manager RN will assess patient needs, develop individualized treatment plans, and maintain ongoing communication with care teams while working in a remote environment.
Key responsibilities
- Conduct thorough assessments of patients' physical, psychosocial, emotional, and financial needs
- Develop and monitor individualized treatment plans in collaboration with patients and care teams
- Advocate for patients by facilitating access to quality care and wellness resources
Required qualifications
- Graduate of an accredited Registered Nursing (RN) program
- Current California RN license required; multi-state license also required
- Minimum of 5 years medical/surgical or acute care experience, including 2 years in case management
- Knowledge of medical claims processing and relevant coding (ICD-10, CPT, HCPCS)
- Proficiency with Microsoft Office Suite and case management platforms such as Eldorado and UM Web
