Job Summary
Guiding patients through complex healthcare journeys, the full-time California Licensed Case Manager RN will assess individual needs, develop treatment plans, and maintain ongoing communication with patients and care teams in a remote setting.
Key responsibilities
- Conduct comprehensive patient assessments covering various needs to create a holistic clinical picture
- Develop and monitor individualized treatment plans in collaboration with patients and healthcare providers
- Advocate for patients by facilitating access to quality care and managing cost-effective strategies
Required qualifications
- Graduate of an accredited Registered Nursing (RN) program
- Current California RN license required; multi-state license also required
- Minimum of 5 years of medical/surgical or acute care experience, including 2 years in case management
- Knowledge of medical claims processing and relevant coding systems (ICD-10, CPT, HCPCS)
- Proficiency with case management platforms such as Eldorado and UM Web
