Job Summary
To support a growing remote practice, the full-time California Licensed Clinical Pharmacist will conduct medication consults, identify adherence barriers, and collaborate with healthcare providers to enhance patient care.
Key responsibilities
- Conduct medication consults with members to meet quality metrics for Medication Therapy Management (MTM)
- Identify and address medication adherence barriers in collaboration with patients' providers
- Participate in team engagements and support patients in therapy initiation and maintenance
Required qualifications
- Ph.D. or Doctor of Pharmacy (PharmD) degree
- Current unrestricted and active pharmacist license in California
- 1+ years of Clinical Pharmacy Practice experience
- Experience with Medication Therapy Management (MTM) and Comprehensive Medication Reviews
- Ability to navigate multiple screens while conducting consultations
