California Licensed Clinical Pharmacist
Job Summary
Engaging directly with members and healthcare providers, the fully remote California Licensed Clinical Pharmacist will improve medication adherence, optimize therapy outcomes, and enhance the overall member experience while working a standard Monday through Friday schedule during daytime business hours.
Key responsibilities:
- Conduct telephonic outreach with members to improve health quality outcomes and medication adherence
- Provide medication counseling and education to support positive health outcomes
- Collaborate with healthcare teams to optimize medication therapy and perform Medication Therapy Management (MTM) services
Required qualifications:
- Bachelor's degree in pharmacy or PharmD
- Active and unrestricted pharmacist license in California and North Carolina
- 3+ years of experience as a licensed pharmacist
- 3+ years of patient consultation experience
- Basic proficiency with Microsoft Office and Windows-based applications
