California Licensed Clinical Pharmacist
Job Summary
Supporting the Multidisciplinary Inbasket Support Team (MIST), the full-time California Licensed Clinical Pharmacist will work remotely to perform medical renewal assessments, prescribe and adjust medications under a collaborative practice agreement, and provide clinical assessments regarding patient home readings.
Key responsibilities
- Collaborate with physicians and healthcare providers to ensure appropriate medication therapy for patients
- Conduct medical renewal assessments and manage medication prescriptions for chronic disease states
- Provide clinical support to providers regarding medication-related inquiries and patient assessments
Required qualifications
- PharmD from an accredited pharmacy school
- Current registration as a Registered Pharmacist within 120 days
- Completion of a Post Graduate Year 1 ASHP General Pharmacy Residency
- At least 3 years of recent relevant experience
- Experience practicing autonomously under a collaborative practice agreement
