Job Summary
To support HEDIS and STAR quality initiatives, the full-time remote HEDIS Clinical Quality LPN will manage medical record reviews, data collection, and provider outreach while ensuring accurate documentation to close quality gaps in care.
Key Responsibilities
- Data mine within electronic health record (EHR) systems to identify necessary documentation for quality gap closure
- Review and abstract medical record information into designated applications and databases
- Conduct outreach to provider offices for medical records and follow up on incomplete submissions
Required Qualifications
- Active LPN/LVN license
- 1+ year of provider outreach experience
- 1+ year of HEDIS and/or STAR experience
- 2+ years of experience with data analysis and quality chart reviews
- Strong medical record review and clinical documentation skills
