Job Summary

Supporting the care management team, the per diem Associate Discharge Planner will coordinate discharge planning and assist with operational functions to ensure efficient transitions for patients from hospital to home or alternative care settings, working remotely during 8-hour day shifts.

Key responsibilities

• Coordinate discharge planning and patient transitions from hospital to home or alternative care settings
• Assist with operational functions of the care management department
• Support the care management team under the supervision of the Director

Required qualifications

• Clinical or administrative experience in the healthcare industry
