Job Summary
Seeking a part-time California Licensed Family Physician, this remote contractor role will provide supervision for Nurse Practitioners and deliver virtual care to Medicare patients with complex health needs, contributing to innovative clinical programs and interdisciplinary collaboration.
Key responsibilities
- Enable best-in-class virtual health care for underserved patients by collaborating with Nurse Practitioners and utilizing internal care delivery technology
- Help develop and refine clinical programs to holistically support patients and address care barriers
- Collaborate with a diverse and inclusive team to provide high-quality clinical care and support care coordination
Required qualifications
- 2+ years of clinical experience serving patients with complex social and healthcare needs
- Board certification or eligibility as a Family Physician
- Eligible to enroll in Medicare
- Medical license in California and active CA DEA or willingness to obtain one
- Experience working with Medicare beneficiaries managing chronic disease
