California Licensed Family Physician

Job Summary

To support a growing practice, the part-time California Licensed Family Physician will provide virtual care to Medicare patients with complex health needs, supervise Nurse Practitioners, and collaborate with a multidisciplinary team while working remotely for 5-10 hours per week.

Key responsibilities

• Enable best-in-class virtual health care for underserved patients by collaborating with Nurse Practitioners and utilizing internal care delivery technology
• Help develop and refine clinical programs to support patients holistically and address barriers in their care
• Work collaboratively with a diverse and inclusive team to provide high-quality clinical care and support care coordination

Required qualifications

• 2+ years of clinical experience serving patients with complex social and healthcare needs
• Board certification or eligibility as a Family Physician
• Eligible to enroll in Medicare
• Medical license in California and active CA DEA or willingness to obtain one
• Experience in Medicaid/FQHC clinics and managing chronic disease in Medicare beneficiaries
