Job Summary
To support a new Longevity Program, a California Licensed Geriatric Nurse Practitioner will deliver telehealth assessments and ongoing medical follow-ups for older adults, focusing on comprehensive geriatric assessments, chronic disease management, and cognitive health in a fully remote setting.
Key responsibilities
- Conduct comprehensive initial longevity assessments covering functional status, cognition, nutrition, mobility, and fall risk
- Provide medical follow-up visits for chronic disease management and ongoing care
- Document in the EHR to support accurate Medicare billing
Required qualifications
- Active, unrestricted NP license in California
- Board certification as an Adult-Gerontology NP (AGNP/AGPCNP) or Family NP with a geriatric focus
- Active DEA registration
- Experience with Medicare documentation and E/M coding
- Comfort with telehealth and EHR documentation
