Job Summary
To support a growing home care team, the part-time California Licensed Home Care Coordinator will conduct in-home care assessments, coordinate participant care, and manage communication with healthcare professionals while working in a hybrid environment that includes both remote and onsite responsibilities.
Key responsibilities
- Coordinate incoming calls related to participants, physicians, and agency services
- Communicate effectively with participants and healthcare staff regarding care updates and schedule changes
- Collaborate with the Home Care Services staff to monitor home care delivery and assist with staffing activities
Required qualifications
- Healthcare/medical licensure or equivalency with at least three (3) years of professional experience
- Bachelor's degree preferred
- Minimum of three (3) years of case management or nursing experience in a clinical or home setting with a frail or elderly population
- Nursing knowledge and training necessary to treat frail, elderly participants
- Experience in care planning and management in a home care environment is ideal
