California Licensed Home Care Coordinator

Job Summary

To support a growing home care team, the part-time California Licensed Home Care Coordinator will conduct in-home care assessments, oversee personal care arrangements, and collaborate with interdisciplinary teams, while enjoying a flexible hybrid work schedule that includes both remote and onsite responsibilities.

Key responsibilities

• Coordinate incoming calls related to participants, physicians, and agency services regarding physician orders and referrals
• Communicate effectively with participants and collaborate with nursing, therapy, and social services regarding care updates
• Assist with staffing and scheduling activities and participate in end-of-life care coordination

Required qualifications

• Healthcare/medical licensure or equivalency with at least three years of professional experience
• Bachelor's degree preferred
• Minimum of three years of case management or nursing experience in a clinical or home setting with a frail or elderly population
• Nursing knowledge and training necessary to treat frail, elderly participants and care for complicated clinical conditions preferred
