Job Summary

To support a growing pediatric practice, the full-time California Licensed Nurse Care Manager will oversee a team of clinical staff, coordinate care for children with chronic conditions, and ensure effective communication between families and healthcare providers in a remote environment.

Key Responsibilities

• Recruit, train, and supervise nursing and support staff while managing performance and professional development
• Coordinate clinical appointments, manage referrals, and ensure compliance with healthcare regulations
• Communicate effectively with patients, families, and interdisciplinary team members to facilitate seamless care delivery

Required Qualifications

• Active California Registered Nurse (RN) license in good standing with a Bachelor's or Master's degree in Nursing
• Minimum 10 years of nursing experience, with leadership experience in a clinical setting
• Proficiency with technology platforms and electronic health record (EHR) systems
• Ability to work collaboratively in a remote environment while managing competing priorities
• Experience working with diverse pediatric populations, particularly in chronic care or care coordination settings
