California Licensed Physician Assistant

Job Summary

To support patients through Chronic Care Management and Remote Patient Monitoring programs, the full-time California Licensed Physician Assistant (PA-C) or Nurse Practitioner (NP) will conduct clinical assessments, develop care plans, and coordinate with a multidisciplinary care team in a 100% remote environment.

Key Responsibilities

• Conduct clinical and health-risk assessments for patients with chronic and complex conditions
• Develop, manage, and update individualized care plans while monitoring patient progress
• Provide patient education and coordinate care with other healthcare providers

Required Qualifications

• Active, unrestricted California PA-C or NP license
• Current national certification (NCCPA, AANP, or ANCC, as applicable)
• 2+ years of clinical experience in primary care, internal medicine, geriatrics, or chronic disease management
• EHR proficiency and ability to work independently in a remote environment
• Reliable internet and a private, HIPAA-compliant workspace
