California Licensed RN Case Manager
Job Summary
To support a growing Enhanced Care Management program, the full-time salaried California Licensed RN Case Manager will manage clinical oversight and documentation for a cohort of approximately 500 members, ensuring care plans meet regulatory standards while working remotely with an interdisciplinary care team.
Key Responsibilities
- Provide clinical oversight and ensure the accuracy and completeness of documentation for care plans and assessments
- Collaborate with healthcare providers and community partners to coordinate comprehensive care for members
- Analyze care gaps and implement quality improvement strategies to enhance patient outcomes
Required Qualifications
- Active California RN license in good standing
- Minimum 2 years of experience as a Registered Nurse, with case management experience preferred
- Strong understanding of Medi-Cal and social determinants of health
- Proficiency in MS Office and electronic health record systems
- Ability to work required hours of 8:30 a.m.-5:00 p.m. PST from a private, HIPAA-compliant workspace
