California Licensed RN Case Manager
Job Summary
Creating and monitoring treatment plans, the full-time California Licensed RN Case Manager will assess patient needs and coordinate care options remotely, working Monday to Friday during designated shifts.
Key responsibilities
- Assess and coordinate the health status and care needs of members, including inpatient and outpatient management
- Develop and implement short and long-term Case Management goals with documented care plans
- Utilize technology to evaluate the cost-effectiveness of treatment plans and monitor health status
Required qualifications
- 5+ years of relevant work experience in clinical nursing and patient assessment
- Bachelor's degree or equivalent work experience related to nursing
- Graduate of an accredited school of nursing with a valid RN state license
- Knowledge of medical terminology and cost containment practices in healthcare
- Ability to collaborate across operational areas within the organization
