California Licensed RN Clinical Coordinator

Job Summary

To provide critical clinical coordination and operational support, the full-time California Licensed RN Clinical Coordinator will manage patient caseloads, ensure compliance with documentation, and support field clinicians in a remote capacity.

Key Responsibilities

• Provide care coordination and clinical oversight for assigned patient caseloads across both agencies
• Review, audit, and sign clinical documentation, including Starts of Care (SOC) and Discharges
• Serve as the point of contact for incoming clinical inquiries from field staff and patients, offering guidance and escalating complex issues as necessary

Required Qualifications

• Active, unrestricted California Registered Nurse (RN) license
• Minimum of 1-2 years of hands-on home health RN experience
• Minimum of 1 year of supervisory experience in home healthcare or a similar field
• Strong proficiency with Kinnser/WellSky EMR preferred
• Ability to fulfill the duties of this role remotely with strong digital communication skills
